Law Enforcement Program: Enrollment
Official Enrollment Request Form and Sample Documentation
When an agency is ready to enroll in the CALEA Law Enforcement Accreditation or Advanced Law Enforcement Accreditation Program, CALEA will send an “Enrollment Package.” The package is addressed to the agency’s Chief Executive Officer and consists of the following items (click links to view sample documents):
- Enrollment Form.
- Legal Basis and Eligibility Statement.
- Law Enforcement Accreditation Agreement (two copies).
- Invoice Options (Program fee schedule).
- Agency Profile Questionnaire (APQ).
- CACE Order Form.
